Before filling out the forms required to register your personal information, please read the Privacy Notification: 

1. By answering the forms respondent  (“RESPONDENT”) consents to the information gathering of PangPamilya.org of your personal data which may include but is not limited to the collection, use, recording, analysis of your personal information (which may include sensitive personal information) to any government entity or any relevant third party for any government-mandated registrations of business entities, and or processing of payroll,  and/or whenever necessary to achieve the lawful purposes or interests of PangPamilya.org  

2. RESPONDENT’S personal information will be held in a secure location. RESPONDENT agrees that  PANGPAMILYA.ORG may disclose personal information about current job standing to other persons, where that disclosure relates directly to the Registration of sponsors and beneficiaries of the relevant RESPONDENT.  The persons to whom PANGPAMILYA.ORG can disclose personal data and information about a RESPONDENT include government agencies. 

3. Without limiting the foregoing, RESPONDENT  acknowledges and agrees to the following: 

(a) During the collection of data, RESPONDENT’S personal information may be collected and used for all purposes directly or indirectly relevant to the Registration of sponsors and or beneficiaries to health care coverage by PANGPAMILYA.ORG.   RESPONDENT’S personal information may also be used for record-keeping of its registration and enrollment of beneficiaries activities to the said agencies or institution, and for compliance with applicable laws and regulations. 

(b) It is PANGPAMILYA.ORG  policy to retain certain personal information of respondents after completing the registrations at least six months from the date of collection. 

(c) Under Republic Act No. 10173 also known as the “Data Privacy Act of 2012” (“DPA”), RESPONDENT has a right to reasonable access to personal information PANGPAMILYA.ORG  holds in relation to RESPONDENT and to have any inaccuracies or errors corrected. If RESPONDENT wishes to access or correct his/her personal information held by PANGPAMILYA.ORG (as permitted by the DPA), he/she should contact at info.pangpamilya@gmail.com

(d) RESPONDENT shall help PANGPAMILYA.ORG observe the requirements of the DPA to keep all personal information relating to Registrations to health care coverage secure and not to misuse such information, or disclose or use the information without due authorization.
